You can find record sheets for physical and volunteering activities for when different people are monitoring you on the Duke of Edinburgh website.

Visit www.dofe.org - go to ’Leaders’

                                   – click on ‘Resources and Downloads’

                                   - scroll down to ‘Assessor pages’, then ‘Activity Log’ 

                                    – Print off the appropriate one.

Parents are reminded that they are not allowed to sign off their childs activity sheet.
